Abstract

Hybrid configuration of the AO/ASIF pinless external fixator in the treatment of compound fractures of distal tibial shafts

The introduction of the AO/ASIF pinless external fixator has allowed the direct conversion of external fixation to definitive internal fixation by intrameduallary nailing in the treatment of compound tibial shafts fractures. This method, however, is difficult for distal tibial shaft fractures. The use of only one clamp in the distal fragment will decrease the stability significantly. To overcome this difficulty, we have attempted to reinforce the single clamp in the distal fragment with a small AO/ASIF external fixator connecting to the first metatarsal bone. In 1998, three cases of compound fractures of distal tibial shafts were successfully treated using this method. (Hong Kong j. orthop. surg. 2001;5(1):58-63)

混合使用AO/ASIF無針及傳統外固定器治療脛骨下幹開放性骨折

對於脛骨幹開放性骨折的治療,AO/ASIF無針外固定器的使用令固定法可以由外固定直接轉為髓內釘固定。但當使用於脛骨下幹開放性骨折時,若下骨折段只能容納一固定鉗,外固定的穩定性會大受影響。為了解決這問題,我們嘗試用一個小AO/ASIF外固定器把下骨折段的固定鉗連接到第一跖骨上。在1998年裡,我們用這混合外固定器治愈了三例脛骨下幹開放性骨折。結果是令人鼓舞的。
