We report a 67-year-old man who presented with cognitive impairment: memory loss, apathy, malaise and generalised weakness. Laboratory, radiological and cerebrospinal fluid analyses were unrevealing until the thyrotropin levels were obtained. Retrospectively the patient had demonstrated the neurological features of hypothyroidism. We review the literature on this subject. (Hong Kong j.emerg.med. 2002;9:102-104)
