A case of type IV injury of the right acromioclavicular joint dislocation is described. Detection of a type IV injury is important because surgery is advised. Computed tomography scanning is necessary to confirm the clinical suspicion of a type IV injury, especially in patients with multiple injuries or those with painful injuries which preclude taking an axillary view or a Stryker notch view.
